'A watershed moment': Pfizer's Covid-19 vaccine is '90% effective' in preventing the illness, company says

The results are in from its phase three trial.

LAST UPDATE | 9 Nov 2020

A VACCINE JOINTLY developed by Pfizer and BioNTech was 90% effective in preventing Covid-19 infections in ongoing Phase 3 trials, the companies has announced today, exceeding the expectations of experts.

The statement was released as soaring coronavirus cases across the world have forced many millions of people back into lockdown, causing further damage to ravaged economies.

European stock markets and oil prices jumped on the announcement.

And US President Donald Trump, who lost last week’s election in part over his administration’s response to the pandemic, hailed the announcement as “such great news”. President-elect Joe Biden, meanwhile, said the news gives “hope” but a long battle lies ahead.

According to preliminary findings, protection in patients was achieved seven days after the second of two doses, and 28 days after the first.

The companies said they expect to supply up to 50 million vaccine doses globally in 2020, and up to 1.3 billion doses in 2021.

“The first set of results from our Phase 3 Covid-19 vaccine trial provides the initial evidence of our vaccine’s ability to prevent Covid-19,” Pfizer chairman and CEO Albert Bourla said in a statement.

“We are a significant step closer to providing people around the world with a much-needed breakthrough to help bring an end to this global health crisis,” he said.

“We are reaching this critical milestone in our vaccine development program at a time when the world needs it most.”

Seeking emergency use authorisation

Across much of the globe, Covid-19 infections rates are hitting record highs, with hospital intensive care units filling up and death tolls mounting.

US biotech firm Moderna, several state-run Chinese labs, and a European project led by the University of Oxford and AstraZeneca are also thought to be closing in on potentially viable vaccines.

Two Russian Covid-19 vaccines have been registered for use even before clinical trials were completed, but have not been widely accepted outside of Russia.

The Phase 3 clinical trial — the final stage — of the new vaccine, BNT162b2, began in late July and has enrolled 43,538 participants to date, 90% of whom have received a second dose of the vaccine candidate as of 8 November.

Pfizer said it is gathering two months of safety data following the final dose — a requirement of the US Food and Drug Administration — to qualify for Emergency Use Authorization, which it expects by the third week in November.

“We look forward to sharing additional efficacy and safety data generated from thousands of participants in the coming weeks,” Bourla said.

‘Watershed moment’

While the Pfizer-BioNTech trial has yet to be peer-reviewed by experts, scientists reacted positively – if cautiously to the results.

Michael Head, Senior Research Fellow in Global Health, University of Southampton, called it an “excellent result for a first generation vaccine”.

Peter Horby, Professor of Emerging Infectious Diseases and Global Health in the Nuffield Department of Medicine, University of Oxford, said Pfizer’s announcement “feels to me like a watershed moment” in the pandemic.

But others pointed out that there would likely be significant logistical problems in getting the vaccine to everyone, especially given it must be kept super-cooled and currently requires two doses to bestow immunity.

Eleanor Riley, professor of Immunology and Infectious Disease at the University of Edinburgh, said for example that the results did not disclose the ages of participants.

“If a vaccine is to reduce severe disease and death, and thus enable the population at large to return to their normal day-to-day lives, it will need to be effective in older and elderly members of our society,” she said.

Dozens more candidates

The so-called messenger RNA, or mRNA, vaccine is a new approach to protecting against viral infection.

Unlike traditional vaccines, which work by training the body to recognise and kill proteins produced by pathogens, mRNA tricks the patient’s immune system to produce viral proteins itself.

The proteins are harmless, but sufficient to provoke a robust immune response.

The study also will evaluate the potential for the vaccine candidate to provide protection against Covid-19 in those who have had prior exposure to SARS-CoV-2, as well as vaccine prevention against severe Covid-19 disease.

Pfizer and BioNTech plan to submit data from the full Phase 3 trial for scientific peer-review publication.

As of mid-October, the World Health Organization (WHO) has identified 42 “candidate vaccines” at the stage of clinical trials, up from 11 in mid-June.

Ten of them were at the most advanced phase 3 stage, in which a vaccine’s effectiveness is tested on a large scale, generally tens of thousands of people across several continents.

No normality yet

The World Health Organisation’s special envoy on Covid-19, David Nabarro, said “any promising news about a vaccine is great news”, but cautioned that there is still some way to go.

He told the BBC: “Everybody who’s hearing and watching this will be saying ‘wow, does this mean that life can go back to normal in the near future?’

“Life will go back to a new normal, and we’re not there yet.

“We do need to be following through on all the basic rules that we now know are important for dealing with this virus – our own behaviour, the way in which governments run their health systems, and also unity between nations.

“And I just want to stress that these principles that we’ve been working for over the last 10 months are still absolutely essential.

“Even if a vaccine arrives in the near future we’ve got many months of still dealing with the virus as a constant threat that we’ve got to make certain that we continue to do all that is necessary to solve the virus causing major problems.

“The vaccine will help, but it’s not going to be a complete game changer.”

US President-elect Joe Biden was equally as cautious and warned that the end of the battle against Covid-19 is still months away.

“This news follows a previously announced timeline by industry officials that forecast vaccine approval by late November. Even if that is achieved, and some Americans are vaccinated later this year, it will be many more months before there is widespread vaccination in this country,” Biden said in a statement.

Experts have said the full effect of a vaccine on transmission in the population will not be known until one is in circulation and more data is collected.

@Pro-Life Eire: That is not how it works. Vaccines overarching goal is herd immunity, herd immunity doesn’t exist without vaccines right, so when enough people are protected typically 70-80% via vaccine, the disease burns out.

You’re also not factoring in the IFR so no 5,000 won’t die even if your scenario was true, in fact covid would burn out quickly.

@Pro-Life Eire: Okay. I’ll bite. Most routine vaccines are between 85 and 95% effective. 90 plus is excellent. If 90% of the population are immune, the likelihood of the other 10% even contracting the disease drops significantly. The very rough estimate of covid mortality is 3-4% of those who contract it.

@Dave O’Keeffe: not sure I get your question? I believe the doctors/scientists who develop the vaccine are absolutely doing their best to create something safe and effective. My non expert, uneducated assumption is that before a vaccine is approved extensive human testing is carried out. However I also assume that when/if billions of people start to take it, some (hopefully not many) will have unforeseen complications, my theory is I would prefer not to be first in the que and that after 12 months one would have some indication of the safety and potential side effects. So for me it is not about believing the doctors, it is about seeing how people get on with it; if that makes sense.

@Pro-Life Eire: I’m sure they won’t release it until it’s fully tested and safe. No vaccine is 100% safe and these are unusual times, we can’t wait 4 years obviously. There’s more money and more companies working on a vaccine than ever before on world history, so we can expect one much sooner than the norm. If you’re happy to wait 4 years, don’t complain about restrictions. That’s my point.

These vaccines have been on no way accelerated or been rushed. Can you show a trying that shows shortcuts have been taken?

How about you make up your mind for you and stop scaremongering and dictating to everyone else. Just a thought. BTW the old and vulnerable are the very ones that prob should not get the vaccine but should be protected by the benefit of everyone else getting it. If the rest of the population do not get the vaccine….it simply wont work but from your comments I see you are a vaccine guru of some sort so I assume that you know this. Wouldnt it be better if we put our collective efforts into ensuring that the vaccine is actually as safe as possible (as we know nothing is ever 100% safe) so we can get on with life rather than trying to put any potential vaccine into disrepute before its even approved??

@Pro-Life Eire: do your homework. Covid19 is closely related to the first Sars virus and they have been working on a vaccine for it for approximately 10 y. Much of the work done from that has been transferred to covid-19, so they weren’t starting from scratch. Because of funding these vaccines will be some of the most trialed and highly monitored vacancies to ever hit the market. By the way the Ebola vaccine was developed quicker and has potentially saved 10s of thousands of lives so far.

It wasn’t rushed. Bo vaccine is tested for long term safety at the outset, patients are monitored after approval.
This virus lent itself to quick testing as somebody else mentioned a lot of work has been done on SARS viruses, it is prevalent in wealthy counties and it is widespread meaning efficacy or non efficacy can be quickly established.
Yes there could be problems with it long term, but that has nothing to do with the speed that testing has happened. Go and read up on it.

One’s chances of surviving COVID exceeds 99% if you are minimally healthy.
The benefit of lockdown is unproven. Living in Sweden without lockdown offers no additional harm.
The chances of children being sick with COVID versus influenza-even with vaccination is appreciable lower.
Are you going to vaccinate your kids under those circumstances? Almost zero benefits and an unknown harm potential.?
Is this vaccination going to be voluntary or compulsory?

Each vaccine will be different in very nuanced ways, but they will ultimately be used in the battle with covid and hopefully all Covid strains.

Great positive news about vaccine – now we need to see the strategy from government about HOW we live with Covid in Ireland getting livlihoods and jobs back along with freedoms to enjoy life – the nanny state are getting a bit comfortable with all their ‘rules and regulations’ and groupthink around Covid – they have done an average job managing the pandemic despite all the spinning – now lets make sure that the road to recovery really is all in this together this time – because the financial impacts on the people in this. country was very very divided and very very far from ‘all in this together’

@Hugo Bugo: Hi Hugo, some of us are more at risk than others and have to be very careful and cocoon because a flu or pneumonia or covid-19 could be fatal and would be fatal for some. Yes, we live ordinary lives, some work and pay taxes but consider the statistics on covid-19 deaths with underlying conditions puts us in the death zone. Some gladly sigh with relief when they get their flu and have their pneumonia vaccine as it means that they can expect to live an ordinary life for another year.

Covid-19 has robbed those of us with underlying conditions of a relatively normal life in part by the people who refuse to wear masks and social distance and by those who refuse to comply with the recommendations to help prevent the spread of the virus.

Some of us have effectively been in level 5 lockdown since last March and welcome a vaccine as a means to help protect ourselves from this virus. If you are young and fit and have a 60 year life expectancy you will of course have a different perspective on a vaccine when compared so people like myself who may have spent last Christmas in hospital on oxygen having been taken by ambulance and spent time in resus on a bipap ventilator.

Now I am NOT a sheep and consider your comments condescending and selfish. If you choose not to take a vaccine that is your choice but I need a vaccine to protect my life from those who choose to take a chance and not get a vaccine and could end up being a potential carrier and spreader.

At least a vaccine which is 90% effective is better than depending upon masks and social distancing and people spoiling Halloween and maybe Christmas by keeping contacts to a minimum to help protect us vulnerable citizens. The more who get vaccinated the more we can open up.

Sadly people do not realise that without a vaccine we will remain with some restrictions and also with deaths for the foreseeable future.

No government has taken my rights away, I chose to work from home a week before it was recommended last March. The people who have taken away my normal life are those who refuse to comply with the measures designed to help stop the spread of the virus.

People need to understand that 90% immunity means almost nobody would get it because the virus would be decommissioned and it would be effectively out of transmission. The 90% who are immune would not pass it on so the chain of transmission gets broken. This news is absolutely super. 90% is massive and it will kill this virus.

@Pro-Life Eire: It’s the same logic as herd immunity if you’d like to simplify it as such but it shouldn’t be compared to herd immunity. Herd immunity would entail leaving the body to its own devices to immunise itself towards the virus, a theory that doesn’t seem to be standing from the research done into it, there is a question mark over herd immunity as a whole and whether or not it actually exists not just in relation to covid prevention. From usage of a vaccine, although only 90% effective it would leave the virus with a limited number of carriers to bring it from one person to another, the likelihood of all unvaccinated carriers being in one place is unlikely, letting the virus die from no host carrier. In theory, It is the idea of herd immunity backed up with man made science

As soon as the rate of vaccination in a population goes under a certain percentage, outbreaks are possible. But a “population” does not mean a country, it could be a small area.

In most places, unvaccinated people ride on the coat-tails of all the other vaccinated people and thus never get the disease at all.

Serious question. I’m horrific at maths. If its 90% effective this means only 90% of those who get it won’t contract covid? The infection rate is currently hovering around 10% of the population. What will change? I don’t get it. I suspect my undetstanding of this must be all arse about face.

@Daff. Myers: Right now we have human to human transmission, and mink to human in Denmark, there is nothing stopping the virus from spreading except the restrictions, if the virus tries to infect a person who has been vaccinated and is immune the virus dies as it has no host infect and then infect others.

So, eventually the 10% who might still get infected will not pass onto others as 90% should be immune and the infection chain 10% of 10% is 1% and the R0 number will drop to .1 where it has been 1.7 in some places.

    Thats my effort anyway
